The TARDIS materializes on Androzani Minor, the cradle of a miraculous life-extending medication meticulously crafted from a compound known as spectrox. The production of this potent drug is meticulously controlled by Sharaz Jek, a disfigured and deranged individual who has self-imposed his own banishment. Blaming Morgus, a formidable industrial magnate of Androzani Major, for his myriad misfortunes, Jek wages a relentless battle against government forces dispatched to liberate the drug.

Armed with weaponry clandestinely provided by gunrunners secretly employed by Morgus, who is compensated with refined spectrox, Jek has established a stranglehold over the drug market on Major.

Enamored with Peri, Jek rescues her and the Doctor from a fate of execution at the behest of General Chellak's government troops. After uncovering their dire plight of having contracted spectrox toxemia—a deadly condition with no known cure except the milk of a queen bat, which the Doctor must retrieve from the treacherous depths of the caves on Minor—they manage to escape. In a desperate bid to stem the erosion of his power, Morgus journeys to Minor.

In a thrilling climax, Morgus, Jek, and all the soldiers meet their end. With time running out, the Doctor hurries Peri back to the TARDIS, where he administers every drop of the queen bat's milk he has been able to gather. Peri recovers, but the Doctor is forced to regenerate to preserve his own life.
